The soap dispenser at the rear hand sink was ripped off the wall, so soap was not available at the sink.
The owner’s children were in the kitchen unsupervised.
One of the urinals was stuck in the flush position.
The critical line of the fill valve was not at least one inch above the overflow pipe in the basement employee restroom toilet tank.
An employee wearing gloves was observed coming from outdoors heading to the salad prep top cooler and putting gloved hands on a box containing food (corrected).
The floor of the basement dough walk-in cooler, walk-in freezer and produce walk-in cooler, microwave oven in the basement private dining room, evaporator fan cover in the basement produce walk-in cooler, door gaskets and surface sealing area of the basement three-door work top prep cooler, storage shelves in all walk-in coolers in the basement, door gaskets and surface sealing areas of doors and interior bottoms of the main level cooler, main level cooking hood and filters were not clean.
There were flies in the bar sink areas.
Other types of seasoning were found in the sugar bulk bin container.
The meatball spoon handle was wrapped with electrical tape.
